To file a Request for Domestic Violence Restraining Order you must submit the following documents to initiate a case:


  • DV-100 Request for Domestic Violence Restraining Order
  • DV-109 Notice of Court Hearing
  • DV-110 Temporary Restraining Order (CLETS-TRO)
  • CLETS-001
  • LF-FAM-105 Mandatory Disclosure of Information (Only required if requesting child custody/visitation orders)


  • To file a Petition for Gun Violence Restraining Order you must submit the following documents to initiate a case:


  • GV-100 Petition for Gun Violence Restraining Order
  • GV-109 Notice of Court Hearing
  • GV-110 Temporary Gun Violence Restraining Order (CLETS-TGV)
  • CLETS-001
  • CM-010 Civil Case Cover Sheet


  • Instructions for e-mail filing a request for Domestic Violence and Petition for Gun Violence Restraining Orders:

    Your filings must be complete and readable.


    You must complete as much information regarding the person you are seeking protection from. If you don’t know the person’s exact age, height, or weight, do your best to estimate.

    Make sure you include your phone number and e-mail address so we can contact you if we have any questions.


  • Each document attached to your e-mail must be a separate attachment in a PDF format. Please use the form number as the name of the document.
  • Check your e-mail frequently, if your documents are not complete, we will return them to you to complete and resubmit.
  • Your Restraining Order will be e-mailed to you, if you prefer to receive it through US mail or to pick it up at the Civil Counter, please let us know. Keep in mind, if we send it to you via e-mail you will need to make additional copies for service.

    After Submitting:

    Once you submit your documents filed by e-mail you should receive a confirmation e-mail from the Court that your submission was received. A clerk will then review your forms. After review, you will receive an e-mail accepting or notifying you that your submission was incomplete. The e-mail accepting your submission will have a pdf attachment with a file-stamped copy of your Request/Petition. If your submission is returned for being incomplete, you will be provided with information as to what additional information is needed.
